will this work in USA?

Yes. I used it for months.. I live in Indiana. >You will have to buy & install a modem & SIM that is USA compatible (NOT INCLUDED). what exactly does this do ? does it send your isp internet through the gsm band to anywhere you have the sim ?

It is a enclosure for a mini pcie cellular modem. You would also need to buy the modem and get a Sim card for your local provider. Then connect it to a computer to get internet access.

do i need a hotspot service for this modem to work or will work fine with data plan?

In my personal experience it works off of data plan and not hotspot. However this is based on a GSM carrier and not CDMA carrier like verizon or Sprint. I don't think that would matter however, the phone tells the network when it's sharing data through hotspot. and this is only a radio like your phone connects to a cell tower. So you shouldn't need a hotspot service in order to use this with your own radio chip.
