Q: is there a back piece or is the back open?

The back is not open. the top fits over the bottom section. The bottom section allows you to change the height of the internal structure (with the use of two pieces of bamboo) so that tissue boxes of various depths will fit into the unit. This is the best tissue box that we have ever purchase, not only is it functional but it looks good too..

Q: Can you use tissues without box? We buy the big boxes at Costco, but when half gone, you have to reach in, and my hands are deformed, so hard.

You mean taking the tissue out of the cardboard box and putting them directly into this? If so you would still have to reach in because the base of the box does not move. However, I think you could make your own base out of cardboard or thin piece of wood perhaps and then insert it into the highest slot (see picture, then your tissue could always be up fairly high like the short boxes.

Q: I need a tissue box cover that will fit the 230 count (2-ply) tissue boxes i buy at costco, 9w" x 4.75 d by 4.75 h. does this qualify?

I have 210 2 ply Kleenex, fits great. LOVE the box!
