A Human Error Approach to Aviation Accident Analysis: The Human Factors Analysis and Classification System

Recently re-published (although remains as a single edition). These authors are the developers of and authoritative experts regarding the HFACS taxonomy of human factor related accidents and incidents. This book (and the related primary literature published by these authors) is a “must-have” and a “must-read” for serious practitioners and students of human factors influence on safety in aviation and other high-reliability industries.

I bought this mostly out of the need to do research for a screenplay and found it to be a very good analytical reference source. The reading is a bit tedious but that's because the authors aren't trying to be anecdotal or even eloquent.

Great read. Streamlines accident investigation and takes it farther than simply identifying what happened, it helps to identify why it happened. The research already captured by the field of aviation is applicable to most skill based professions.

This book provides a simple to understand system to classifying human factors as they relate to aviation accidents. If you are an accident investigator in any other discipline this book should help you.

Great book offered at a good price.

I learnt about HFACS in my Human Factor course during the Master of Science in Aeronautics Program and now I'm addicted to this framework. Every investigator should use this tool to unveil the real reasons why human errors occurred. And more, I met Dr. Shappell, one of the author, and he is an amazing person.

The delivery was a bit slower than expected, but the book arrived in excellent condition.

The book was very useful to work on my diploma paper. An excellent review of human factor models.
