Q: What is the diameter of the pipe bolt holes?

I'm not 100% sure of the actual size but I drilled a 1/2" hole in my concrete floor thru this hole and there was still room for a larger size if needed.

Q: What is the diameter of the screw turning rod hole?

The hole is 7/16" on mine, but there is a burr around the edges that might be troublesome for a rod of that diameter. The rod they supply (which is very short), fits perfectly though and it's exactly 3/8".

It was used to replace a wooden support post in a basement. It has been up for a few years now with no problems. Worth the money

It depends on the floor support system. If you have a load-bearing beam. Put one where the beam terminates at the wall and another under the safe - on the beam. I have two in the garage under a hot tub that is on an upstairs deck.
